Spring Athletics Physicals

The Spring Athletic Physicals are scheduled for Saturday, February 20 from 9:00am to 11:00am at Allegheny General's Century III HMR office.

Location: Century 3 HMR, 1st floor

Date: Saturday, February 20, 2010 (Additional dates are listed below)

Time: 9:00 am to 11:00 am

Cost: $10.00 (cash or check made payable to AGH)

Reminder:

One complete physical is required by the PIAA to participate in high school athletics.

The physical examination can be completed by your family physician or by AGH on the above date.

Please complete Sections 1,2,3 of the PIAA form and the AGH HIPPA/Consent to Treat Form.

Physicals will last approximately 45 minutes, please wear comfortable clothing.

Additional reminder:

In order to have a physical examination completed, the HIPPA form must be signed by a parent or legal guardian, or athlete if they are over 18.

If a student-athlete plans on trying out for Baseball, Boys Lacrosse, Girls Lacrosse, Softball, Tennis, or Boys Volleyball, a completed athletic physical is due in the athletic office by Monday March 8, 2010 by 3:00pm. If you played another sport this school year, Section #5 of the PIAA Physical (The Re certification Form) is needed to be completed and signed.

Saturday, February 13 at AGH Bethel Park (9:00am to 11:00am)

Wednesday, February 17 at AGH Suburban (6:00pm to 7:00pm)

Wednesday, February 24 at AGH Federal North (6:00pm to 7:00pm)
